If you have a Logan County Football game below is a screen shot of the link that they will send you to set you up for payment.  Complete the top portion of the form.

When you select the Sport and Level the below box will appear for you to select the game that you are assigned to.  Do not worry about tehamount per the level, you will be paid what is in Arbiter pay not what is on the drop-down box.  This was created before the pay rate was adjusted in Arbiter.
